Who can Pre-Register?
- Patients with a procedure already scheduled who wish to Pre-Register online
- OB Patients who would like to pre-register for Labor and Delivery
- Patients who need to request a screening Mammography appointment
Before you can pre-register, you will first need to enroll to create your online profile. Once you enroll and set your password, you will receive a confirmation email, similar to what happens when you sign up for online banking. This password will then allow you to pre-register online.
Pre-register or begin the enrollment process if you are first-time user.
Please pre-register online at least 48 hours before your visit to the hospital to allow adequate time for processing your information.
What you'll need to Pre-Register:
- Your personal contact information including occupation information
- Emergency contact information
- Insurance information, group name and number, policy number, address and telephone number
- Physician name
- Date of procedure
Once at the facility, just look for the kiosks with the orange button above, or any one of our patient registration staff, to register.
